perform general dentistry services, there are a few participating specialists
including periodontists, oral surgeons, orthodontists
and endodontists.
Reduced fee procedures include: Emergency
exams, Emergency treatment for pain, X-rays, Initial and periodic exams,
Cleanings, Fluoride Treatment, Sealants, Extractions, Fillings, Dentures,
Partial Dentures. Dentists may charge
their regular fees for dental procedures not included on this list.

Medicare is a federal
health insurance anyone 65 or older, anyone under age 65 who has
been ruled disabled for 24 months and any kidney dialysis or kidney
transplant patient.
People who are not automatically enrolled should apply for Medicare by
contacting any Social Security Administration office. In order to avoid waiting periods and/or
higher premiums applications should be sent in at
least three months before the applicant turns 65.
Free Care at Hospitals and
Depending on the
patient’s family income, health centers and hospitals are
expected to provide emergency, in and outpatient medical care at no cost
or on a sliding scale. Doctor bills are
usually NOT covered.
Eligibility: Patients whose family income is 200% or under
the federal poverty level, patients whose family income is between 200 – 400%
of the federal poverty level are eligible for free care, if paying a hospital
bill will cause financial hardship even if the family income is greater than
400% of the federal poverty guidelines, the patient may be eligible for free or
partial free care.
To apply for free care, request an application from the hospital,
health center or clinic. There is no
limit to the number of hospitals or health centers to which patients can apply for free care, but each one requires a separate application.

ACT NOW Plan (
ACT (Access to Care and
Treatment) Now provides comprehensive primary health
care services for people who have HIV and who are uninsured or inadequately
insured. Services are
provided at a variety of sites including neighborhood and community
health centers, group medical practices and hospital based infectious disease
clinics. An important goal of this
program is to maximize the health of people living with HIV;
to improve their quality of life, and limit or avoid the use of emergency room
and other high-cost clinical services. Populations targeted by this program include patients with a
history of substance abuse, patients whose first language is not English and/or
whose cultural and health-related background is different from those of the
dominant culture, patients newly released from jail, patients with a history of
mental illness, and patients who live in communities that are geographically
isolated from AIDS care facilities.
ACT NOW pays up to $1,500 per client per year.

staff person must act as advocate/intermediary for patient in the application
process. Advocate calls for or downloads

patient qualifies a 30-day supply of prescription medication is authorized immediately. Advocate then mails application to Glaxo. Once
verified, an additional (max) 60-day prescription will be approved. Separate procedure if drug will be ongoing

service to the people of the Commonwealth.
